Operational Health Assessment
This is not a quiz and not a report card. It produces a work order: one scored number, eight scored areas, and a single indicated place to intervene first.
- Who completes it
- The owner-operator. If you have a manager or second-in-command, have them complete a separate copy independently.
- Time to complete
- 8–12 minutes. Do not deliberate. First-instinct scores are more accurate than considered ones.
- Timeframe
- Rate each item based on the last 90 days.
- Retest
- 90 days after intervention begins. Same instrument, same rater, no reference to prior scores until after completion.

Compare an owner copy against a manager copy
Each completed assessment produces a result code. Paste two codes here to see where the leadership team is not seeing the same business. A spread of more than 8 points on any section usually outranks whatever the scores themselves say.
The treatment target
Two rules override the total. The Bottleneck Rule: any single section scoring 16 or higher is the treatment target regardless of total burden — one area, not all eight. The Capacity Rule: if Owner Capacity scores 20 or higher it is addressed first no matter what else scored higher, because an operator in that range cannot execute any downstream intervention.
Section scores
The vertical line marks 16 — half of the maximum, and the threshold at which a section becomes the treatment target.
Each section is scored out of 32. Total out of 256.
Reading the pattern, not just the number
For pattern purposes, a section is read as high at 16 or above and low at 8 or below.
Defense & Repair
A cross-cut of items you already answered across five sections, scored as one construct: what the business is exposed to, and whether it heals from what gets through. It does not change the total burden, the band, or the treatment target.

Use notes
- The instrument produces the work order, not a report card. Score → identify the bottleneck → intervene at that area only → retest at 90 days. A section that has not moved by retest means the intervention was wrong, not that more time is needed.
- Compare owner and manager copies before discussing results. A spread of more than 8 points on any section means the leadership team is not seeing the same business. That finding usually outranks whatever the scores themselves say.
- Score before the discovery conversation, not during it. Owners revise their answers in real time when a consultant is watching.
- Retest data is the proof asset. Three clients with documented pre/post burden reductions is more persuasive than any case study written in prose.
